Our Community Navigators are here to help you access local services and resources that support your health and wellbeing. Whether you’re feeling isolated, struggling financially, or looking to get more active or connected, we’re here to listen and guide you.

Community Navigators

As community navigators, our role is to support individuals in accessing local resources and services to improve their health and well-being. We act as a bridge, connecting people with the right support, whether it’s social activities, education, or information about local services.

  • Need help getting into employment or training

Community Navigators give you time to focus on what matters to you by taking a holistic approach to your health and wellbeing. They connect you to community groups and statutory services for practical and emotional support which includes:

  • Money and debt advice
  • Benefits
  • Emotional wellbeing support
  • Volunteering, training and employment support
  • Social groups and activities
  • Bereavement support
  • Lifestyle e.g. weight, smoking and alcohol and substance misuse
  • Staying independent
  • Family support
  • Support around a disability

What Do We Do?

  • Community navigators build trust and relationships with people and service providers, fostering a sense of community and mutual support.
  • They empower people to take control of their lives and make informed choices about their health and well-being.
  • By connecting people to the right resources and support, community navigation contributes to improved mental and physical health, greater social inclusion, and enhanced quality of life.
